Vital Records

How to obtain a vital record through Sheffield:

It is recommended that you contact the department to first verify that we have your record. Once that has been established, there are options to request/obtain vital records in Sheffield. You may call the office at 413-229-7000, ext 151; you can email at: townclerk@sheffieldma.gov; or you can order through the Town’s website payment link.

• Please be mindful that birth records are available through the Hospital town of the birth, or in Sheffield, if the parents, at that time, lived in Sheffield.

• Marriage records will be available in the Town that you filed Intentions, not the town you married in (although they can be one in the same).

• Death records are available if the decedent passed away in Sheffield or if their residence was in Sheffield at that time.
